Forms andinstructions

Find a number of important forms for the Part-Time MBA degrees and classes with helpful instructions and deadlines. There are forms and instructions for everything from tracking your progress toward graduation, to getting on the waiting list for a particular course, as well as the application you need to complete to graduate.


Graduation Planner

Graduation Planner is an online tool for students to use as a guide in planning their academic career. The MBA Student Affairs Office recommends that students submit a plan at least twice during their academic career. The recommended time frame is to submit the first draft after completion of most core course requirements, and then submit a final draft when approximately 10 credits remain in their program.  You may submit revised Graduation Plans as needed.


To submit your plan for review by the Part-Time MBA Registrar, please send an e-mail to Program Coordinator, Jessica McLain, with the subject "Graduation Plan for (Student Name)" and include the name of your plan, your student ID number and your expected graduation date in the body of your e-mail.

Application for Degree Form(PDF 148k)

Part-Time MBA students are eligible to graduate any month of the year. This University of Minnesota-wide form initiates the graduation process and should be sent directly to the MBA Student Affairs Office.


  • The deadline for your Application for Degree is the first working day of the month in which you intend to graduate. For example, if you want to graduate in December, your form must be sent in no later than the first working day of that month.
  • If graduating at the end of Spring or Summer semester, please turn in your Application by March 1st, in order to ensure that you get commencement invitations and cap and gown ordering information in a timely manner.
  • Please submit your Application for Degree by faxing it to the MBA Student Affairs Office at 612-626-7785, or dropping it off in person at Suite 1-110.

Waiting List Instructions

If a class is full and you wish to be added to the waiting list, visit the One Stop registration system, log in, and add yourself to the desired waiting list.


Non-Carlson School students do not have the option to place themselves on a MBA course waiting list. Please see instructions on requesting enrollment.


Please note that the MBA Student Affairs Office controls the waiting lists for Part-Time MBA courses; therefore disregard information on the One Stop website regarding permission (or magic) numbers and instructor approval.


Student priority on the waiting list is determined on a first come, first served basis. If a space becomes available, Peter Woxland will enroll you in the course and contact you.


Non-MBA graduate Student Registration Form (PDF 393KB)

Non-MBA Graduate students:

  • Can petition to take up to 3 Carlson MBA courses
  • Students are registered on a space-available basis, a few weeks before the term starts.
  • If you are enrolled in a course, you will receive a confirmation email from the MBA Student Affairs Office.

Petition for Day MBA, HRIR or Non-MBA Classes

Part-Time MBA Students wanting to register for Full-Time MBA, HRIR or classes outside the MBA program:

  • Part-Time MBA students can petition to take Full-Time MBA elective courses and HRIR courses on a space-available basis.
  • Part-Time MBA students can also petition to take courses outside the MBA program.
